Your treatment of the torn paper looks good.
You should sand between layers of filler.
Keep adding filler and sanding until you can't feel the patch with your bare fingertips.

Not a silly question at all.
You can buy kits to test for lead. But if the house was built in the 60s, there's a very good chance that there's lead in the paint. USA didn't ban lead paint until 1977.
