I need to mount a 15-pound medicine cabinet on some old 1970's drywall. (At least the product page says it's 15 pounds, but I have a 14-pound dog and it feels considerably heavier - maybe 20/25 pounds)...

At what point does hanging items on the interior side of an exterior wall become a concern regarding damage to the insulation?

Take a large toggle bolt. That can break the vapor barrier and compress the insulation. Or at least that's what I'm thinking. Small anchors should be okay, no?
