The difficulty is that there are alternative possible wiring schemes for the existing lights. Is the line power coming through the switch and then to the lights or directly to the lights with a switch loop (or a combination among the existing lights)? Pictures of the wiring in the existing switch boxes and the boxes holding the lights may give us enough info to help.

There is construction adhesive, such as PL, which is very strong. Many other brands. Just make sure you don't plan to remove it (can be done but often destructive).

A drill press could do it fairly easily. If you don't have access to that equipment, you could use a coping saw. Or you could glue a small patch of the same thickness to the bottom edge that is bigger than the hold, drill the hole and then trim off the added patch.

Good point. If you don't have access, plan as close as possible, then fill the gap on the fireplace side with caulking.

Growing up in NYC where nothing but armored was allowed, and then Long Island where everyone knows (?) you can't have exposed NM (and raceways were the only surface treatment allowed), this is an eye opener. I think I agree with @DMoore that the building inspectors I have met would insist on NM being behind the walls except in unfinished space. But, as you say, the code does not seem to explicitly require it.

Not likely. Looks like a pot metal casting, and a very thin body at that point. Seems to be held in place by pressure pins. Theoretically the pressure pins could be forced out and a new arm inserted, but much more trouble than the hinge is worth. @Paulster2 's comment seems the best approach.
